HUCFM also offers:

Low THC Oil Registry Cards
HUCFM offers Low THC Oil Registry Cards to patients who are at least 18 years of age and members of our Pear’d Direct Primary Care program.
Pear’d is a monthly membership program designed to provide you with greater access to Primary Care than the insurance system does. It costs $100 per member per month, and includes unlimited Primary Care with no co-pays, co-insurance, or hidden bills.
To be approved for the registry, you’ll need to have an initial evaluation with one of our practitioners. It’s up to the practitioner to determine whether or not you have a condition that meets the requirements of the registry. The fee for the office visit is non-refundable.
You must continue to be a Pear’d member, or your card approval will be rescinded.
Low THC Oil is considered for patients with the following conditions:
- End-stage cancer, or when the treatment produces related wasting illness or recalcitrant nausea and vomiting
- End-stage or severe amyotrophic lateral sclerosis
- Seizure disorders related to epilepsy or traumatic head injuries
- End-stage or severe multiple sclerosis
- Crohn’s disease
- Mitochondrial disease
- End-stage or severe Parkinson’s disease
- End-stage or severe Sickle cell disease
- Severe Tourette’s syndrome
- Autism spectrum disorder, when the patient is (a)18 years of age or more, or (b) less than 18 years of age and diagnosed with severe autism
- Epidermolysis bullosa
- End-stage or severe Alzheimer’s disease
- End-stage or severe AIDS
- End-stage or severe peripheral neuropathy, when symptoms are severe or end stage
- Patient is in hospice program, either as inpatient or outpatient
- Intractable pain
- Post-traumatic stress disorder resulting from direct exposure to or witnessing of a trauma.
